Previous 'childbirth' vaginal, c/s etc ….. My first childbirth was vaginal and not instrumental, and my end childbirth was vaginal too and instrumental, with forceps
Did you want to / plan to breast feed ….. I would like to breastfeed my
baby and to have she with me.
Have you breast fed before, if so for how long ….. I breastfeed my first baby during 3 months more or lest. I would like to do it more time.
Labour and birth

Were you induced? ..... yes with “ocsitocina”
Date of delivery? ..... My baby was born 3 jun 2002 about 16:40h. She not breathe. She have an “distocia de hombros”. My baby was very ill and she was in the incubator. My baby died 13 jun 2002
Problems during delivery? ..... My childbirth was vaginal wih forceps. No cesariana.
If emergency caesarian why? were you happy with this? .....
Problems after delivery? ..... After the childbirth I was>operated for vital urgency. I had an internal bleeding. I was in >pain, but the doctors had not anything. An anaesthetic doctors find out what was my problem, and I was carried to the operating theatre.

2. Did you have an epidural? Did you have any problems with your epidural? ... Yes. I have not problems. I was in pain after the childbirth.

4. Were you aware of any precautions made for the delivery eg saving blood units
/ not allowing a birth in the 'home suite', being given 'senior' midwives for
the delivery? ... (not understood)

5. Were you allowed to eat / drink and if not what reasons were given? ... (not understood)
6. What procedures were followed immediatley after delivery? ... (not understood)
7. When did you realise that there was a problem? ... I know the problems 3 days after.
8. Was your husband allowed to stay in the delivery suite whilst they attempted
to stem the bleeding? ... No, my husband had in the incubator with my baby when the pediatrician told he I had an bleeding.
9. What was done to stem the bleeding? ... Anything.
10. Did you have any bi-manual compresions and if so did you have any pain relief? ... I don´t understand. I was in pain.
11. Were you conscious whlist they attempted to solve the problems? ... This doctors have not know to solve the problems. I think they have caused the problems. They must have a cesariana to me and not to force the vaginal childbirth.
12. Did you ever feel as though you were not going to survive? ... No I was worried for my baby.
13. Did you feel as though everything (ie the staff) were completely in control
of the situation? ... No they have not know what it has happened.

No they say I had all the uterus front broken and they say have not can to conseve it. but the doctor boss say me the have can to conserve it sewing it with an special thread, but He don´t write this in her report.

Do you know, have you met the doctor that did the operation?
Was the doctor a consultant? ..... Yes, but only one moment. The doctors did me the histerectomy are not the same that were in the chilbirth. They must be in the chilbirth but here were only the horrible midwife and the student doctors.
Was an internal review undertaken to see hat happened and if mistakes coul be
learnt from? (Anna was told this was standard practice with such an outcome)
..... No I thik this is only important for meand not for the hospital
Did you think about having your organ (s) returned to you? Any results? ….. No I have thinking this but I have not had anything about.
Have you obtained your notes? Were there any problems? ..... Yes, I have had problems but I have the clinical story.

..... No they not write all. There are a lot of notes wich lack.
Has the doctor gone over your notes with you? ..... no
Did you have all your questions answered? If so was this straight away
or did you have to go though some form of complaints procedure? Sucessful?

If you wish to meet or speak to the pepole involved in your 'care' are
you able to? ..... No I speak with the girl had me the forceps but she only cry and she not say me anything.

When did it HIT you? ..... The worse for me was and is the death my daugther.
Did you plan to be an organ donor before the hysterectomy? Are you still? Or
vice versa. ….. I don´t understand.
Has it effected family life, if so how? ..... I try it
How has your husband / partner dealth with this? ..... My husband try to do a normal life.
How has this changed the quality of your life? ..... Now I try to live. After I lived.
What are your overall views? ..... I think my life has changed. We was a family with normal problems, a happy family . And now we are some persons that they try to be a family.And I think we will never pull trough this experiencie.
With hindsight would you have done anything differently? ..... Yes the doctors should have done me a “pelvimetria “. And they should have done me a cesariana. I should have chosen an experience doctors. If the doctors have to me a cesariana at least my baby was alive and well.
Advice for others ….. .. I don´t know what advise to give the other persons. Only I wish them the best.
